Indiana University School of Medicine will confer degrees upon members of your class, the 105th graduating class, on May 9, 2009. (The first class to graduate from IUSM was the Class of 1907. Two separate classes graduated in 1942 and again in 1944 due to the accelerated need placed on the School by World War II. NRMP Match Results will be available on Thursday, March 19th. The results cannot be released to students until 12:00 noon EST. Hard copy match notification letters will be distributed to student in the atrium of the Medical Science Building beginning at noon. Match results will also be available on the Web at NRMP's Web site (http://www.nrmp.org) beginning at 1:00 pm on March 19. Students will need their AAMC ID number and password in order to access the results on the Web. A Class party is scheduled for that day following distribution of the Match letters. (You will be excused from your elective or clerkship for the day).
Students can learn their matched/unmatched status (but not their residency assignment) via the Web beginning at 12 noon EST on Monday, March 16. Unmatched students may begin contacting unfilled programs at 12 noon EST on Tuesday, March 17. Faculty and staff of the Medical Student Affairs Office will work closely with those students who do not match to help secure a residency position.

Only those who have met the above requirements will receive their diplomas at Commencement on May 9. Students completing graduation requirements in May or later should make arrangements either to pick up their diploma or to have it shipped to them via certified mail.
PLEASE NOTE THAT STUDENTS ENROLLED IN MAY COURSEWORK CANNOT RECEIVE THEIR DIPLOMAS AT COMMENCEMENT ON MAY 9. THIS INCLUDES STUDENTS ENROLLED IN ANY ADDITIONAL ELECTIVES OVER AND ABOVE THE SEVEN REQUIRED.

Each state varies slightly as to its licensing process, so it is advisable that you check with the appropriate state board of licensure regarding the state process and law. You may also contact the Federation of State Medical Boards (FSMB) for general information concerning licensure and for the addresses and phone numbers of individual licensing authorities: FSMB, 400 Fuller Wiser Road, Suite 300, Euless, TX 76039-3855, telephone: (817) 868-4000. The United States Medical Licensing Exams (USMLE) must be used for licensure. Indiana University graduates will have passed USMLE Steps 1, 2CK, and 2CS as part of their graduation requirements. Passage of Step 3 is required to complete the USMLE licensure requirement. Step 3 is typically taken during the first year of residency.

To be eligible for NBME / USMLE licensure certification, an individual must:
Eligible individuals will have the option of scheduling to take each Step throughout the year, with quick rescheduling possible, at a location selected from many available computer-based test centers. Residency programs may require sitting for Step 3 by a certain date. Students should discuss the Step 3 examination with their residency program administrators.

Temporary Medical Permits (TMP’s) may be obtained from the Medical Licensing Board of Indiana immediately following graduation, and are required for the post-graduate training done prior to obtaining full licensure. Applications for TMP’s must be requested from the IPLA. TMP’s will not be granted on a walk-in basis.
To apply for a TMP, you must submit to the Medical Licensing Board of Indiana:
An Indiana Medical License may be obtained from the Medical Licensing Board of Indiana by certified graduates after successful completion of one year of approved post-graduate training in the U.S. or Canada and successful completion of the USMLE Step 3.
Controlled Substance Registration Permits (CSR Permits) are issued through the Indiana Professional Licensing Agency (see: http://www.in.gov/icpr/webfile/formsdiv/34617.pdf). Full licensure is needed in order to obtain a CSR permit, however both licensure and the CSR permit may be applied for at the same time. Contact the IPLA for further information.
Drug Enforcement Administration Permits (DEA Permits) information will be provided by the Drug Enforcement Administration, Resident Office, 575 North Pennsylvania Street, Suite 408, Indianapolis, IN 46204 (226-7977). DEA Permits are required after the permanent license and Controlled Substance Registration Permit have been received. An application for the DEA Permit may be submitted approximately one month prior to receiving the license, as the processing time for the Permit is about eight weeks. The current cost of the license is $551.00 for a three-year permit. The form (224) may be accessed at www.deadiversion.usdoj.gov.

Students who have received the Federal Primary Care Loan (PCL), the Federal Perkins Student Loan, the Federal Loans to Disadvantaged Students, the Federal Stafford (Sub and Unsub) Loans, the Federal Graduate PLUS Loan, any School of Medicine Loans, the Mable Sledd Cunnison Loan, or the Robert Wood Johnson Loan are required by federal regulations to have an exit interview before graduation can be certified. Detailed information regarding the required Exit Interview Session held on the morning of MATCH DAY will be e-mailed to you in February 2009. The information you will receive at the mass exit interview session conducted on the morning of March 19 before receiving your match results will include your total borrowed loans for the four years of medical school as well as information on your loans from the Sallie Mae Web site and the National Student Loan Data Service (NSLDS). A completed Exit Interview (an online process) as well as any other required forms from the IU Student Loan Administration (for those to which it applies) must be on file before your graduation can be certified. Students who received a long-term School of Medicine loan will also be required to complete an Exit Interview, as well as any forms required by IUPUI. IU School of Medicine long-term loan recipients will receive pertinent exit interview requirements specifically for these loans in the Exit Interview Session packet handed out on March 19th. All members of the Class of 2009 are strongly urged to participate in the Commencement/ Convocation program on May 9, 2009. NOTE: Students enrolled in coursework after April 2009 will not receive their diplomas during the Commencement exercise, but are eligible to participate in Commencement, and are encouraged to do so. The diplomas will be available after May 31st, upon completion of the last scheduled course and receipt of the final evaluation form, and verification that all other requirements have been completed satisfactorily. Each year the Association of American Medical Colleges (AAMC) asks that students graduating from medical schools complete the AAMC Graduation Questionnaire. The information collected from the survey is used to improve medical education on national and local levels. Please logon to http://www.aamc.org/gq. You will be directed to a secure site where you are asked for a Token ID. You will be receiving an email regarding the Token ID from MECA. The site will provide a link to the Web site if you want to save the survey and enter back into the system to complete at a later time. As guaranteed by the AAMC this new login process is anonymous for the Program Evaluation Survey portion of the GQ. The second survey on Financial Aid, Career Intentions and Diversity requires you to login using your AAMC ID Number as this portion contains personally identifiable information.

As you know, one of the requirements for graduation is to "complete evaluations of all elective courses taken during the fourth year" (Student Handbook). In an effort to expedite the completion of these forms, the Dean's Office for Medical Education and Curricular Affairs has made them available via the Web.
Students do not need to fill out elective evaluations for courses not in the IU Elective Catalog. These course numbers all end in 990 and are exempt from this requirement.
